Edward parasites adopts the name “Nefariamous,” a name bestowed upon him by Satan himself. As the plot unfolds, Martin, a skeptic, dismisses Nefarious as a mere ploy by Edward to feign madness. However, Martin is taken aback when Nefarious vehemently demands his own execution, urging Edward to meet the same fate. Beware, for the demon’s treachery knows no bounds.
A man named Dr. Stewart, who was a psychologist, sadly took his own life by jumping off a building. Another psychologist named Dr. James Martin comes to a prison to talk to a prisoner named Edward, who is a famous serial killer on death row. Dr. Martin was asked to take over for Dr. Stewart, who used to visit Edward regularly. Edward wants to be executed, but it’s up to Dr. Martin to determine if he is sane or not. If Edward is declared sane, he will be executed, but if he is declared insane, he will be spared.
When Dr. Martin meets Edward, something strange happens. Edward insists that he is not human, but a demon named Nefariamous living inside his body. Edward says that Satan gave him the name “Nefarious” for short. Dr. Martin thinks Edward is pretending to be insane, but he gets surprised when Nefarious insists on being executed. Nefarious also tells Dr. Martin that he will commit three murders before the day is over.
Dr. Martin doesn’t believe Nefarious and thinks he is making it all up. Nefarious gets angry and calls Dr. Martin names because he doesn’t believe in demons. Nefarious explains that he started tempting Edward when he was young, and then he possessed his body after Edward agreed to it. Dr. Martin brings a chaplain to confront Nefarious, but it doesn’t work because the priest doesn’t believe in demons.
Later, Edward breaks through and begs Dr. Martin for help, claiming that the demon has been using his body to kill people. When Martin investigates, Nefarious takes control again and accuses Martin of murdering his own mother. Martin gets shocked because Nefarious knows about his mother’s death, but he explains that he made a difficult decision to euthanize her because of her terminal illness, not for money.
Nefarious tells Martin that he mentally tortured Dr. Stewart until he committed suicide so that Martin would take over. He also reveals that Martin’s girlfriend is terminating her pregnancy because Martin threatened to leave her. Martin doesn’t believe Nefarious and refuses to make a phone call to save the unborn child.
A prison orderly asks Edward for his final meal, but Nefarious changes the order. Nefarious then explains his perspective on the war in Heaven and wants Martin to write a book called “The Dark Gospel,” which will create a new world religion serving demons. He also tells Martin that more people are going to hell than to Heaven.
Warden Moss discovers evidence linking Martin to Nefarious and urges him to condemn Edward. Martin confronts Nefarious, who breaks free from his restraints and threatens to harm him. Martin begs for his life, and Nefarious spares him. But the prison guards subdue Nefarious.
During Edward’s execution, Martin feels uncertain about his decision. He realizes that Nefarious’s prediction of three murders might have been his mother, his son, and now Edward. Just before the switch is pulled, Nefarious briefly takes control of Edward’s body and asks Martin to accept his offer, but Martin refuses silently. Edward suffers a painful death in the electric chair.
One year later, Martin has changed and promotes Nefarious’s book as a cautionary tale about demonic influence. He encounters a homeless woman who speaks with Nefarious’s voice, suggesting she is the new host for the demon. The screen goes black with Nefarious’s laughter heard.
